A 2027 U.S.–China conflict with Australia aligned to the U.S. China trade stops immediately, Taiwan exports are slowed/limited, and China launches large‑scale cyber disruption in waves. Planning scenario, not a forecast. No end date is set.

What happens

Why U.S. device makers and their hospital buyers get hit

  1. Step 1 is the China slice only. China was 7.96 percent of 2024 core-device customs value and supplied $5,595,773,620. Mexico was 21.11 percent and supplied $14,838,272,063. Ireland supplied $8,365,705,978. Germany supplied $7,339,226,832. Costa Rica supplied $5,633,713,916. The line stops when a China-origin component, syringe, needle, or commodity device is missing.

  2. Step 2 is what still ships. In 2025 Mexico was 21.28 percent, Ireland 12.38 percent, Germany 10.16 percent, and Costa Rica 9.33 percent. Costa Rica rose 22.6 percent from 2024. China fell to $4,994,334,593 and 6.74 percent, a 10.7 percent decrease. The top five countries were over 59.3 percent of core imports. Singapore, Switzerland, Japan, and the Dominican Republic also remain on the 2024 top-ten list.

  3. Step 3 is electronics, and it is not a Taiwan share. AdvaMed said device makers were less than one percent of the chip market and were 18 percent of responses to the Commerce semiconductor request for information. Two-thirds of companies had semiconductors and firmware in over half of their products. Needs are 2nd or 3rd generation chips, competing with automotive, industrial, and consumer uses. Delays vary significantly, from two to 52+ weeks. No opened page prints Taiwan's share of chips inside a U.S. device.

  4. Step 4 is the gate. Volt Typhoon pre-positioning is the infrastructure baseline for ports, energy, and telecom that devices move through. The assignment of device gateways to Wave 1 is scenario inference, not a count of device-plant attacks.

    cisa.gov

  5. Step 5 is the buyer and the payment file. There are 6,100 hospitals. Participating hospitals had self-assessed adoption of 70.7 percent of NIST Cybersecurity Framework functional areas. A clearinghouse failure is a different node from the factory. Change Healthcare produced estimated losses of $874 million.

How hard, how soon

0–3 months

  • Short-horizon consumable analog: the determination raised the additional rate to 100 percent on China-origin syringes and needles, applicable on or after September 27, 2024. This is not a year-over-year index peak.

    ustr.gov

  • Short-horizon basket analog: February 2020 was 142.8 and March 2020 was 144.1. The calculated one-month move was 0.91 percent. The annual supplies print is not this band.

    data.bls.gov

  • Short-horizon payer clock, labeled as a funding term and not as an outage length. For repayment, providers will have 30 days to return the funds. GAO did not print 2 days. The loss analog remains estimated losses of $874 million at Change Healthcare, a health payment processor, not a device plant.

    unitedhealthgroup.comgao.gov

1–6 months

  • Mid-horizon time analog: delays vary significantly, from two to 52+ weeks. The portion of that range inside one to six months is the planning band. It is not a printed price index.

    advamed.org

  • The page's China ocean matrix of 20-40 days sits at the edge of the first quarter, not in a year-over-year price. It is a sourcing-matrix lead time, not a Census series.

    meddeviceguide.com

12+ months

  • Annual buyer analog, not the one-month PPI: that instrument bill rose 7 percent from 2023, and total supplies expense increased 9.9 percent in the 2025 growth figure. Supplies are not devices only. The long end of that same range is 52 weeks.

Wave 1

Critical infrastructure that moves devices: ports, customs, energy, and telecom. Inland trucking is indirect to that wave.

cisa.gov

Wave 2

Enterprise systems: device plants, merchant-wholesaler platforms, and payment clearinghouses. Hospital buyers and in-hospital biomedical staff are indirect to this wave.

gao.govbls.gov

Wave 3

Distributed firms: independent repair shops, medical-equipment rental firms, and small clinic buyers.

What to do

Day 7

  • Split open orders into China-origin versus Mexico, Ireland, Germany, and Costa Rica. The China baseline to tag is $5,595,773,620 of 2024 core customs value, 7.96 percent of that book.

    meddeviceguide.com

  • Flag syringe and needle SKUs under 9018.31.00 and 9018.32.00, and separate enteral syringes, because the additional duty determination raised the additional rate to 100 percent.

    ustr.gov

Day 30

  • Ask each device line whether chip delay is already past the short end. The short end of that delay range is 2 weeks. Qualify the alternate part while the long end, 52 weeks, is still a planning case.

    advamed.org

  • Hospital buyers name the installed base that biomedical staff can keep alive. Medical equipment repairers held about 71,800 jobs, and the hospital median wage was $75,390.

    bls.gov

Day 90

  • Put a manual order path next to the clearinghouse. The observed analog is estimated losses of $874 million at Change Healthcare, with widespread provider impact.

    gao.gov

  • Recheck the instrument budget against the printed buyer baseline. Hospitals spent over $10 billion on surgical and medical instruments, and that instrument bill rose 7 percent.

    aha.org

What to watch

By role

Device manufacturer

  • Keep domestic and Mexico, Ireland, Germany, and Costa Rica lanes loaded.

  • Dual-source any China-origin component and any mature chip on lines already inside the 2-week delay.

  • Separate enteral syringes from the 100 percent syringe and needle subheadings.

meddeviceguide.comustr.govadvamed.org

Importer and wholesaler

  • Tag the $5,595,773,620 China slice at SKU level.

  • Put safety stock on the wholesaler bench that employs 33 percent of repairers.

meddeviceguide.combls.gov

Hospital buyer

  • Track the instrument bill against the over $10 billion baseline, not against the $297 billion supplies line.

  • Keep biomedical staff on the installed base of monitors, ventilators, and imaging.

  • Write a paper claim path for a clearinghouse outage.

aha.orgbls.govgao.gov

Independent repair and rental

  • Stock the parts that failed in the two-to-52-week shortage.

  • Price service off the rental median of $45,020 and the occupation floor below $38,490, which is thin cash.
