A 2027 U.S.–China conflict with Australia aligned to the U.S. China trade stops immediately, Taiwan exports are slowed/limited, and China launches large‑scale cyber disruption in waves. Planning scenario, not a forecast. No end date is set.

What happens

Why Australian medicines get hit when China trade stops

  1. Step 1, the finished-dose invoice stays with other partners. China was 1.1 percent of finished-dose import value on the calculation above, equal to 68,361.06 thousand dollars. The United States, Germany, Ireland, and India are the four largest HS 3004 origins. A China stop does not, on this table, remove those four.

    wits.worldbank.org

  2. Step 2, antibiotics and a slice of bulk medicaments are the China lines. China was 36.9 percent of antibiotic import value. China was 15.4 percent of bulk-medicament import value. France, Italy, the United States, and India remain on the antibiotic page. Italy remains the bulk leader at 20,814.51 thousand dollars. They are continuations in the customs sense. They are not printed as volume replacements for the China line.

  3. Step 3, Indian finished doses are a real Australian origin, at 520,104.75 thousand dollars of HS 3004, and they are not a clean API substitute. Brookings, writing about the U.S. generic market and India's trade statistics, says the dollar share of Chinese imports relative to all API India imports has been hovering around 68% plus or minus 2%. That 68% figure is not an Australian customs share. It is the upstream limit on treating Indian packs as China-free.

    wits.worldbank.orgbrookings.edu

  4. Step 4, Taiwan. The four opened 2024 Australian WITS partner tables do not name Taiwan. No PBS, AIHW, Guild, or stockpile page opened here prints a Taiwan medicine quantity. The scenario's Taiwan export restriction is an explicit limit for this industry.

    wits.worldbank.orgwits.worldbank.orgwits.worldbank.orgwits.worldbank.org

  5. Step 5, the cash path is PBS claiming, not a domestic plant census. As of 30 June 2025, 99.9% of approved suppliers of PBS or RPBS medicines used online claiming. A claiming outage stops payment in the same week the terminals stop. That is the short-horizon operational fact. It is not a printed outage duration.

  6. Step 6, hospitals and the stockpile. In 2024-25 there were 703 public hospitals and 68,200 public hospital beds. In 2023-24, $113.8 billion was spent on hospital care, of which $90.8 billion was public hospitals and $23.0 billion was private hospitals. The stockpile holds specialised medicines and pandemic influenza antivirals and can direct CSL Seqirus to manufacture pandemic influenza vaccine if a pandemic is declared. Requests go through the state or territory Chief Health Officer. It is not a general supply for a China antibiotic stop.

How hard, how soon

0–3 months

  • Short-horizon facts, and the limit on price: 99.9% of approved suppliers of PBS or RPBS medicines used online claiming, so a rail outage is a same-week cash stop. No day count of such an outage was printed. The ABS page says the Health group rose 3.7% over the past 12 months and does not print a pharmaceutical-products series, so that 3.7% is not used as a 90-day medicine price. No one-month PBS or pharmaceutical CPI print was opened. The day-count analog is the regulator clock, not a counted shortage of a named product: no later than 2 working days after discovering the shortage. The shortest cash window is worth $49.97 million over July to November 2025, which is five months and is repeated in the next band.

    guild.org.auabs.gov.autga.gov.au

1–6 months

  • Five-month pharmacy cash: 8.05 million dose administration aids worth $49.97 million from July to November 2025. Full-year aids were 18.65 million, valued at $115.52 million. These are service payments, not a medicine price index.

    guild.org.au

12+ months

  • Full-year payer prints: Government expenditure was $19.1 billion and was up 7.4% from 2023-24. The average dispensed price was $91.62, up from $85.10. Calculated from those two printed prices, the average dispensed price rose 7.7 percent. Health group rose 3.7% over 12 months. None of these is a 0-3 month medicine price.

Wave 1

Wave 1 is ports direct, public hospital pharmacies direct, and the National Medical Stockpile indirect. The ACSC fact sheet says state-sponsored cyber actors target Australian businesses and critical infrastructure, and health care and social assistance was 6% of incident reports.

cyber.gov.auhealth.gov.au

Wave 2

Wave 2 is enterprise supply and payment. Finished-dose importers, antibiotic importers, and online PBS claiming systems are direct. The PBS and RPBS payer is indirect because payment depends on the claim. A measured foreign payer incident is labeled as that incident, not as an Australian pharmacy loss. Change Healthcare, a health payment processor, had estimated losses of $874 million. For repayment, providers will have 30 days to return the funds. That 30 days figure is the US payer's funding term.

guild.org.aupbs.gov.augao.govunitedhealthgroup.com

Wave 3

Wave 3 is the community pharmacy set, 6,013 pharmacies, direct, and concession and general patients, indirect. The ACSC small-business cost of $56,600 is the fetched small-firm cyber cost, and it is not pharmacy-only.

What to do

Day 7

  • Match the China antibiotic line to 14,331.37 thousand U.S. dollars and 265,342 kilograms, and the China finished-dose line to 68,361.06 thousand dollars. Do not treat the finished-dose line as the main loss.

    wits.worldbank.orgwits.worldbank.org

Day 30

  • Confirm supply from the United States, Germany, Ireland, and India on finished doses, and ask Indian suppliers where the API is made, against the Brookings figure of hovering around 68%. Test a paper claim path because 99.9% of approved suppliers claim online.

    wits.worldbank.orgbrookings.eduguild.org.au

Day 90

  • Qualify antibiotic kilograms outside China against 265,342 kilograms. Do not book the National Medical Stockpile as PBS cover. Its quantities are unpublished and it is not a general supply service. Track the average dispensed price against $91.62 and the subsidised script count against 226 million.

    wits.worldbank.orghealth.gov.aupbs.gov.au

What to watch

By role

Finished-dose importers and sponsors

  • Keep purchase orders with the United States, Germany, Ireland, and India, the four largest HS 3004 origins.

  • Separate the China finished-dose line of 68,361.06 thousand dollars from the China antibiotic line of 14,331.37 thousand dollars.

wits.worldbank.orgwits.worldbank.org

Antibiotic importers

  • Do not treat India's 7,435.76 thousand dollars as a replacement for China's 14,331.37.

  • Ask whether Indian API is inside the Brookings hovering around 68% Chinese import share.

wits.worldbank.orgbrookings.edu

Community pharmacies

  • Plan manual dispensing for 6,013 pharmacies if claiming fails.

  • Watch the community book of around 327.2 million prescriptions separately from the 226 million subsidised count.

guild.org.aupbs.gov.au

Public hospital pharmacies

  • Prioritise antibiotic continuity for 703 public hospitals and 68,200 beds.

  • Do not request stockpile product except through the Chief Health Officer, and do not expect formulary cover.

aihw.gov.auhealth.gov.au

PBS and RPBS payer

  • Track expenditure against $19.1 billion and the average price against $91.62.

  • Keep the $18.6 billion AIHW subsidy figure as a second series, not a revision of $19.1 billion.
